Each staff member has met the State of Virginia’s educational and training requirements for the services they provide. We encourage, support and require continued education of each service provider.
A Registered Nurse will first conduct a preliminary evaluation of the type of services needed in your home. Family members, physicians and other health care professionals will work together to develop a customized plan of care. The care plan includes details about the services to be provided, it's frequency and duration of care. The care plan is reviewed weekly and is updated to meet your changing needs.
Team Nursing Approach - The Team Leader completes the initial assessment and matches the patient to a caregiver ensuring compatibility and continuity.
Cardiopulmonary Assessments - Highly experienced Registered Nurse with a minimum of 3 years clinical background including vast knowledge in the care of cardiothoracic surgery, cardiopulmonary and cardiovascular patients.
Preventive Measures and Disease Management - Improve health outcomes, identify and reduce unnecessary hospital readmission through care that is available, responsive, proactive and compassionate.
Patient Education - Emphasis on actively engaging in turn enabling them to make informed decisions regarding their care.
LVAD (Left Ventricular Assist Device) - Site care and management
POC Testing for INR: - A Skilled Nurse will perform testing between 7a.m. – 10a.m. and have results available before 12 noon for Coumadin dosing in the evening.
On-Call Services: - In addition to an RN Team Leader, a member of the Administrative Team consisting of Registered Nurses is available on-call 24/7 including holidays to provide resource and support.
